Is there a troubleshooting guide somewhere?
Set manager for all 3 roles to issue templates. Under administration, information:
Environment:
Redmine version 2.3.1.stable.11984
Ruby version 1.8.7 (i686-linux)
Rails version 3.2.13
Environment production
Database adapter Mysql2
Redmine plugins:
redmine_issue_templates 0.0.5
Problem: Don't see any options for templates when click on issue or new issue despite being manager. Don't know if it's a compatibility issue (seen issue for 2.3 problem), or need to do something to get first template in place, or messed up the install, or what?

I don't know how I missed it... but didn't check the module under project, settings, modules, issue templates. Most other things had screen shots, and I somehow skipped over that one instruction under plugin installation.

Let me confirm you have selected issue templates plugin as project module or not.
I've just add instruction to activate this as project module.
If any error after submitting, please confirm the migration task.
